A: No, we do not provide medical care at Valley Health Connections, but we can help you find a provider. We can help you establish with a provider at North Star Health or one of the local primary care clinics. There are also free clinics located throughout VT which you can find here https://vtfreeclinics.org/

Q: What should I bring with me to my appointment?

A: For most of the services we provide, proof of income is needed.
Here is a list of what we typically need:

  • Most recent tax return (if you file).

  • Current years Social Security Benefits Statement (if you receive benefits).

  • Social Security numbers and Dates of Birth for all household members.

  • Two of your most recent bank statements.

When you call to set up an appointment, staff will make sure to tell you what you need to bring with you. If you are not sure how to access needed documents, we can help you with that as well. If you don't know how to access a document, do not let that stop you from reaching out to us.

Q: I live in New Hampshire (or another state), can you still help me?

A: While we are based in VT and help with mostly VT programs, we can help you with some services if you live in NH. We can still help you with Medicare and financial aid. If you need help with health insurance (outside of Medicare) you can call the NH Navigator line at 603-931-3858 or visit them at https://acanavigator.com/nh/contact
